SOCIAL SERVICES
Jewish Family Services provides a wide range of SOCIAL SERVICES including:
- Information/Referral related to Jewish communal and public sector services
- Development of an individualized service plan to meet a senior’s and/or caregiver’s specific needs
- Linking seniors with caregiving support to ensure safety and stability
- Coordination of all designated services and monitoring of continuity of care
- Advocacy and intervention with community resources on behalf of a senior or caregiver, to ensure a secure network of services
- Short-term counselling – support and guidance for seniors and their families in coping with transitions and challenges associated with aging.
fees: $90.00/hr or sliding scale for lower income families; minimum cost $5.00/hr
- Assistance with the application process and transition to retirement residences and long term care homes.
- Crisis Intervention
- Family Link - support to local and long distance family, paid, and unpaid caregivers
